When input requires specific formats or patterns (e.g., phone numbers, dates), instructions are provided to inform the user of the expected format.


Missing form-level instructions:

The “From” and “To” date fields require mm/dd/yyyy, but no instructions are provided at the beginning of the form. Users must guess the format and may trigger validation errors.
